It’s easy to refer a patient to our service, and our referral guide tells you everything you need to know, including our referral criteria.

Referrals at-a-glance:

  • Complete a referral form and fax to 0333 321 8844 or email it to:

Please ensure you include the following details:

Patient information
Name, address, contact number including mobile, DOB, sex, ethnicity.

GP Information
Name, surgery ID and address, telephone number, fax number, nhs.net email address.

Clinical information
Type of scan request, patient clinical history, supporting clinical information, questions to be answered.
